The following forms are to assist you in the process of changing, amending or managing your investment.

Regular Savings
If you would like to initiate regular savings, change account details, change your regular savings amount or cancel your Direct Debit please download the Direct Debit PDF form.

Additional Contributions
If you would like to make a lump sum deposit into your plan please download the Additional Contribution PDF form. The completed form should be returned to us with your personal or bank cheque.

EasyClaim/Withdrawals
If you wish to make a claim for education expenses or a withdrawal for other reasons, please download the EasyClaim PDF form. If you are claiming for education expenses or living expenses over the annual 'living allowance' limit, please provide supporting documentation.
